If you are a first time caddie, please complete the following:
Caddie Info
- Step 1
Download and print the Ozaukee Caddie Manual and Permission FormComplete the last page to turn into the golf shop.

Caddies between the ages of 12-15 must apply for a work permit on Wisconsin's Department of Workforce Developmentwebsite. Once completed, the DWD will send the work permit to Ozaukee Country Club. If you receive it at home, bring the work permit to any caddie training for record keeping purposes.
- Step 4
Download and print the Caddie Exam.
- Step 5
Turn in all the last page of the caddie manual, the caddie information page, your work permit, and the caddie exam into the Golf Shop. Please be aware that you must have them all completed before you are able to caddie.
- Parent information
Our goal is that each caddie, due to the time they put in during each training session, will be able to explain questions you might have regarding our program. If not, please download the Parents-Guardians Informational Guide which will explain how our caddie program operates, provide important information regarding the Chick Evans Scholarship, and other insightful tips to help you as a parent better understand our caddie program.

Returning Caddies, if you have a work permit on file at Ozaukee Country Club, you are not be required to turn in another work permit.
